Check out Formulize 2.1 RC1 here. There's a link to the developer's website for downloads.

Formulize enables admin to create custom forms with text fields, check box, radio button etc. Admin sets access permissions for each form.

Submitted forms appear in the admin module as, e.g.

submitter - - - colour - - - price
----------------------------------
Tobi - - - - - - Green - - - 28
Suzy 
- - - - - - Black - - - 45


Admin can perform calculation on a group of submitted forms (e.g. total all the 'price' fields) and export to CSV file for a spreadsheet.

Very powerful. Not easy to learn (IMHO)... but I am a little dim sometimes

I am looking for forms with MySQL backend. I do not have big expectations from this type of Module but if it can give me basic form to gather data for about 10 fields, I will be happy.

I tried form generator modules in repository but could not find any.. does this mean nobody needed so far !! If I have acted like a blind, please point me to right place.
